Coaching Services Acknowledgment and Agreement
Trident Mountain, LLC D/B/A Everyone Deserves To Heal

By engaging in coaching services provided by Trident Mountain, LLC D/B/A Everyone Deserves To Heal, I acknowledge and agree to the following terms:

1. Nature of Coaching Services

I understand that the coaching I receive is not a substitute for professional mental health care, therapy, or medical treatment. Coaching does not diagnose, treat, or cure mental health conditions or medical issues. I recognize that my coach is not serving as a licensed mental health professional or medical provider.

2. Licensing & Jurisdiction

I acknowledge that coaching is currently an unregulated industry, and my coach is not licensed by the State of Colorado. For all legal purposes, the services provided by Trident Mountain, LLC D/B/A Everyone Deserves To Heal will be considered to have taken place in the State of Colorado.

3. Personal Responsibility

I agree that I am fully responsible for my own physical, mental, and emotional well-being, as well as for the decisions and actions I take during and following each coaching session. I understand that coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment, and I will not use it in place of such services.

4. Purpose and Informed Consent

I understand that all feedback, insights, and ideas shared by my coach are intended solely to support me in reaching the personal goals I define. I confirm that I am capable of providing informed consent and freely give my consent to engage in coaching services with the intent of personal growth and development.

5. Confidentiality and Limits

I understand that the information I share with my coach will be treated as confidential unless I give written consent otherwise. Exceptions to this confidentiality include:

• If I disclose abuse or neglect of a child, elder, or vulnerable adult.

• If I express intent to harm myself or others.

• If my coach is required by law or court order to disclose information or testify.

Additionally, I recognize that while electronic communication (email, phone, Zoom, text, etc.) is often used in coaching, it is not always secure. I accept the associated risks to confidentiality when using these technologies.

6. Cancellations and Scheduling

I agree to provide reasonable notice if I need to cancel a scheduled session. Missed sessions without proper notice may be forfeited at the discretion of the coach.

If I am receiving coaching through Trident Mountain, LLC D/B/A Everyone Deserves To Heal, I understand that the hours provided must be used within 90 days of the start date. Any unused hours after this period may be forfeited.

7. Release of Liability

I release and discharge Trident Mountain, LLC D/B/A Everyone Deserves To Heal, including my coach, their employees, contractors, representatives, and any affiliated agents, from any and all claims or liabilities that may arise as a result of the coaching relationship. This includes any perceived failure to achieve personal outcomes or any consequences resulting from decisions I make during or after the coaching engagement.

I affirm that this agreement contains the full understanding between myself and Trident Mountain, LLC D/B/A Everyone Deserves To Heal, and no additional promises or agreements exist outside of this document. This agreement is binding upon me, my heirs, legal representatives, and assigns.
